Exam Certification Cover Sheet

The Exam Certification Cover Sheet serves several purposes:

1. Provides the Education Officer (EO) with specific guidelines for how the exam is to be administered.
2. Provides official certification that the exam was administered by the EO in accordance with the professor’s instructions.
3. Provides verification that the person taking the exam is the student.

Without this cover sheet the EO may be at a loss for how to administer the exam and/or may administer it incorrectly, and without the certification signature there may be no legal recourse should irregularities appear later. Please take a moment to fully complete this cover sheet and submit it with every quiz and exam.
